Select your desired form from the lists below. The form will
open in Adobe Acrobat Reader. With the mouse, position the pointer
(hand) inside a form field, and when it changes to an I-beam,
click and type your text. For a check box, when the hand changes
to a pointer, click to enter a check mark.
After typing your text or checking a box, press Tab to accept
the information you entered and to move to the next field. After
typing the last field, press Enter.
Complete all fields, and review the form to verify the accuracy
of the information entered and to correct any mistakes.
Print the completed form, sign the proof of service in
blue ink, make the appropriate number
of copies, and submit the original
and one copy to SERB for filing.
